This is still present.

Note, if the world is running on a Windows PC, the same PC does NOT have this issue, only friends who connect to the world.
We'll reset the world voxel map, but this bug still exists after 5+ years. Also, it appears it is related to where meteorites hit the ground.

Does the issue only occur for clients/friends who connected to this world/server, and does the same voxel display correctly for the host/you?
Did the issue occur on an asteroid created by the Voxel Hand?
Was the world/server modded (with active MODs) where this issue was occurred? If so, could you please check on a vanilla world/server (with no active MODs) to see if the issue persists there as well?
If it does, could you please send us a screen recording or video showing the issue, the world save file where we can replicate this issue, and the log file from the time the issue occurred?

After further research into this issue, the cause is due to the large VX2 file and the client not being able to stay in sync, thus the Streaming icon constantly showing on the client. You can reproduce the issue on a client by having a planet of 34+ MB in size, have a large hole (like 8x8 drills going down a few pistons deep). Disconnect and then reconnect after the world has been running on the host for a day. The next time the client begins to load in the world is new looking, the hole takes about 30 seconds to show up. During that time you can fall into the hole and be underneath the ground layer space.
It appears the modular encounter system has some sort of ability, though I've never seen it happen, to despawn and restore the world voxel back to the way it was. I've also read that many years ago there was a cleanup tool that built in that restored the planet where meteorites struck the ground, but no mention on if it is still built in or how to access\use it.
It appears SE needs to auto enable a cleanup system if the meteor showers are to be used, or else it's best not to use them, which takes away from the fun.
Perhaps the voxel changes could be consolidated every so often... For example, if you dig a hole or cave you could have hundreds or a hundred thousand voxel changes. If you then looked at which changes were connected you could then optimize the voxel changes into a smaller number of changes by combining them into larger changes. Another way to think of it if the world was made of cubes. You dig out 10,000 cubes and have 10,000 changes, but you could optimize those 10,000 changes down to say 10 changes by combining them.
There's all sort so of ideas\solutions due to this being a common issue.
