So far, we haven’t been able to reproduce the bug under our testing conditions, which makes it quite difficult to pin down. However, we've created an investigation ticket to keep digging into the issue.

I am still getting this issue. It seems to only be the Factorum storage facility. It happened twice after a server reset but it has now happened to a newly spawned Factorum encounter. Also all of the base was there except that the base door was gone.
This is on a Keen server #3.
