We play on Steam/Pc and the same thing happens when we access other servers. Every time we exit and enter the game, the items on us disappear.

I am sorry you are experiencing this issue. Can you please provide me with more information? Are you using any scripts or mods in your game? When did this issue started to happen? Are you and your friend playing on the same wifi or are you in different locations? Is it only you and your friend experiencing this problem or have you heard of other players experiencing it as well?
Can you and your friend please try the following:
Verify your game files in Steam
-Right click on Space Engineers in Steam Library → Properties → Installed Files → Verify Integrity of Game Files
Delete SpaceEngineers.cfg file
-You can access your SpaceEngineers.cfg by typing %appdata% into your Windows search bar and you will be redirected to the hidden Roaming folder. After that just follow: \Roaming\SpaceEngineers.
- It means your game configuration will be deleted (you can make a backup somewhere on your computer in case it does not work)
Make sure your operating system is fully updated
Also, please share your log file with me if none of the above work.
