There is a "bump" of some sorts at the bottom of the Short Stairs which prevents a clean mating with Grated Stairs and prevents a player from walking up. How to produce: Short Stairs up top, with a Grated Stairs continuing the stairway below. Attempt to climb up the stairs. You will get stuck at the Short Stairs intersection.
