I would probably switch 1 and 2, (I agree with 3)
It would just be so much faster scrolling through them instead of pressing r

I also like this idea. I found myself automatically trying to scroll through block shapes. I'd switch CTRL to whatever keybinding ends up also doing block rotation (the suggestion elsewhere for the rotation system), but this makes sense to me. So I'd suggest:
1. Scrollwheel - select hotbar slot
2. (rotation-key)+Scrollwheel - change block shape
3. CTRL+Scrollwheel - change hotbar

Also since I left it out, because Ctrl + Scrollwheel currently changes how far away your block is, that could probably be swapped to Shift + Scrollwheel

And because why not I'll add this here too, R should change the size of the block so instead of scrolling R and Shift + R change the size, like how it changed between large and small grid in SE1
