Non-seamless map area respawned?

Not sure how/why. I was drilling though this "rock" and the cobalt was beneath it, and the drill suddenly got stuck, so I went inside the hole and tried to dig it out, and it (the drill) exploded eventually.
However I tried to get out of the hole... and could not.
I uploaded the the save game from inside the hole. If you try to exit by following the piston it will not let you out unless you randomly drill around trying to get out and when you get out you'll see something like this with the screenshot of the piston somehow embedded in the rock face as though the rock had respawned.
