Planetary mining - gaps in voxels

v2.2.0.2113 - Classic Survival, balanced mod installed
While mining underground, I keep noticing gaps in the voxels. See the attached video for an example with multiple gaps. The gaps appear to show the surface, but I know that I am underground far beneath a sloped surface. I'm right up on a wall between iron and dirt/stone. (I keep picking up Nickel when not mining Iron).
Not sure if this relates to falling through the planetary surface.
Second attached video shows the hole and hill that I'm digging into, to show that it is indeed underground.
I do know that if I keep mining near those spots, the gaps will sometimes disappear when the voxels break.
I have the same bug Access denied
