Restore projecter-replaced blocks to the grid's toolbar

When a block gets replace via being projected, it's function should be restored to the toolbar. Currently, that only happens if the item in the toolbar config was a group, but only certain functions make are presented as part of a group, so you can't always do it. Also, the now-phantom item in the toolbar doesn't show a name (again, unless it was a group) when you mouseover.
